Agreement to Terms

By booking an appointment or using the services of Bodhi Tree River Massage Therapy, you agree to these terms of service. Please read them carefully before scheduling your session.

Service Description

Bodhi Tree River Massage Therapy provides professional massage therapy services by Sahra Rogers, Licensed Massage Therapist (LMT #33.024115) in Boardman, Ohio. Services include therapeutic massage, relaxation massage, and specialized techniques as discussed during consultation.

Appointment Policies

Scheduling

• Appointments are scheduled by phone at (330) 553-9675
• Online booking may be available through our scheduling system
• First-time clients should arrive 15 minutes early for intake forms
• We will confirm appointments 24-48 hours in advance

Cancellation Policy

• Cancellations must be made at least 24 hours before your appointment
• Same-day cancellations may result in a cancellation fee
• No-shows will be charged the full session fee
• We understand emergencies happen - please call as soon as possible

Late Arrivals

• Please arrive on time to receive your full session
• Late arrivals will still end at the scheduled time
• Sessions may be shortened if you arrive more than 10 minutes late

Payment Terms

• Payment is due at the time of service
• We accept cash, credit cards, and debit cards
• Gift certificates are available
• All sales are final - no refunds on completed services
• Prices are subject to change with 30 days notice

Health & Safety Requirements

Client Responsibilities

• Provide accurate health history information
• Inform us of any changes to your health status
• Communicate your comfort level and any discomfort during treatment
• Follow any pre- or post-treatment instructions
• Do not come for treatment if you have a fever or contagious condition

Contraindications

Massage may not be appropriate for certain health conditions. Please inform us if you have any acute injuries, infections, blood clots, or other medical concerns. We may require physician clearance for certain conditions.

Professional Boundaries

• All massage therapy is provided in a professional, therapeutic setting
• Appropriate draping will be maintained at all times
• Any inappropriate behavior will result in immediate termination of service
• We reserve the right to refuse service to anyone

Limitation of Liability

Massage therapy is generally safe when performed by licensed professionals. However, you acknowledge that massage therapy involves some inherent risks. You agree to hold Bodhi Tree River Massage Therapy and Sahra Rogers, LMT harmless from any injuries or adverse reactions that may occur during or after treatment, provided services are performed within the scope of practice and professional standards.

Privacy & Confidentiality

All health information and treatment details are kept confidential in accordance with professional ethics and Ohio state law. Please see our Privacy Policy for detailed information about how we protect your personal information.

Gift Certificates

• Gift certificates are valid for one year from purchase date
• Lost gift certificates cannot be replaced
• Gift certificates are non-refundable but may be transferred
• Services must be used by the expiration date

Modifications to Terms

We reserve the right to modify these terms at any time. Changes will be posted on our website and take effect immediately. Continued use of our services constitutes acceptance of any modified terms.

Governing Law

These terms are governed by Ohio state law and regulations for massage therapy practice. Any disputes will be resolved in Mahoning County, Ohio.
